MATERIAL AID

List of documents to be submitted by the applicant to receive material aid:

1. The completed application;
2. A copy of the applicant’s identity card;
3. A copy of the child’s/children’s birth certificate;
4. Civil status documents (marriage, divorce, death) (if necessary);
5. A certificate confirming the family’s income or proof of registration as unemployed;
6. The child’s disability certificate (if necessary);
7. The decision placing the child in a family placement service (where applicable);
8. The lease or loan-for-use agreement, or the homeowner’s statement granting free use of the dwelling (where applicable);
